The shape of the buttocks changes with age and these changes are usually defined by loss of volume, skin elasticity and projection. Gluteoplasty is a procedure that uses implants, transplanted fat or a combination of both to fill in missing volume and correct a flat or sagging buttock.
Patients without sufficient body fat can be good candidates for buttock implants, while in most other cases fat grafting is preferably performed alone or in combination with implants (composite or also hybrid gluteal augmentation).

Buttock shaping
Butt augmentation and modeling with autologous fat (Brazilian butt lift)
Butt augmentation with autologous fat, also known as a “Brazilian butt lift”, allows the size and shape of the buttocks to be adjusted by injecting the body’s own fat. Excess fat from areas such as the abdomen, back, hips or thighs is removed, processed and then injected specifically into the buttocks region to achieve a more youthful and plump shape.
In this procedure, the fat cells are removed from the donor area using special cannulas, carefully prepared and implanted back into the buttocks through fine cannulas. This lipofilling technique, in which the body’s own fat is transferred, can also be used in other areas of the body to increase volume, such as the face, lips or breasts.
As this is the patient’s own fat, there is no risk of a rejection reaction. The transplanted fat cells integrate into the tissue of the recipient area and take on its characteristics, making the result look and feel natural.
Butt augmentation with implants
Buttock lift
In a buttock lift, excess skin is removed and the underlying fatty tissue is used to improve the contour. In contrast to the Brazilian butt lift, in which fat is transferred without removing skin, the aim of the lift is to optimize the shape by removing excess tissue.
This process can smooth ripples and dimples down to the buttock crease at the thigh junction and shorten an elongated buttock crease, resulting in a more youthful shape. The procedure can be performed in isolation or as part of a more comprehensive body lift procedure after significant weight loss, known as a lower body lift or circumferential abdominoplasty.
A combination with implants is also possible to further improve the shape of the buttocks and achieve a full, youthful appearance.
Surgery, ability to work, risks and aftercare
Buttock surgery is performed on an outpatient basis under local anesthesia, twilight sleep or general anesthesia and usually takes 2 to 3 hours. Patients can usually return to light physical activity after 5 to 7 days.
If performed correctly, risks such as swelling, bruising, wound healing disorders and infections can be minimized. Complete recovery and reduction of swelling takes around 2 to 4 weeks. Aftercare includes wearing a compression garment for 6 to 8 weeks, sleeping on the stomach for 2 to 3 weeks, showering after 3 days and a break from sport for approx. 6 weeks.
Costs for buttock surgery
The costs for the procedure are discussed in detail with the patient in advance and are to be understood as a lump sum to be paid in advance. Included are the costs for anesthesia, use of the operating room, implants and all other associated expenses, with the exception of costs for compression garments and overnight accommodation.
